Complete the Referral Passport form above — it takes around five minutes and captures placement criteria only (no personal identifying data). Once submitted, it is matched against live, CQC-regulated supported living vacancies across England, and you choose which matched providers are contacted.

Any registered health or social care professional with authority to initiate a placement search. This includes social workers, hospital discharge coordinators, care managers, occupational therapists, community mental health nurses (CMHNs), learning disability nurses, CHC case managers, resettlement managers, ICB commissioning teams, brokerage teams and independent case managers.

The Referral Passport collects non-identifying placement criteria across ten sections: (A) referring organisation and team, (B) person profile — age, sex and primary need category, (C) care and support requirements, (D) risk level and known risk flags, (E) funding type and weekly budget, (F) mobility and access requirements, (G) preferred location and search radius, (H) accommodation type preferences, (I) additional clinical notes and documents available, and (J) professional declaration. No names, NHS numbers, dates of birth or addresses are collected at any stage.

All major adult supported living need categories: learning disabilities (LD), mental health (including Section 117 aftercare placements), acquired brain injury (ABI), autism spectrum conditions (ASC / ASD), physical disabilities, dual diagnosis, Continuing Healthcare (CHC), substance misuse recovery, and older adult frailty. Both registered and unregistered supported living settings are matched.

As soon as you submit your Referral Passport, it's matched against live, CQC-regulated vacancies and you can review your shortlist straight away. From there you can either have the platform email your shortlisted providers automatically, or contact them yourself by phone. We can't control or guarantee exactly when an individual provider will reply, but referrals marked as urgent are flagged so providers can prioritise their response.

Yes, completely free. There is no cost to submit a Referral Passport, no registration required, and no ongoing commitment. The service is funded through the providers listed on Find Your Room.

No personal identifying data is collected during the referral submission. The Referral Passport captures placement criteria only. Full care documentation — care plans, risk assessments, CHC Decision Support Tools, MCA assessments and so on — is only shared with shortlisted providers under appropriate data-sharing agreements, and only with your knowledge and consent.
